By signing this document, the Authorised Signatories named in this document request and agree to the following:

I/we request the following actions related to the individuals named in "Add Signatory 1",  "Add Signatory 2", and/or "Remove Signatory 1", "Remove Signatory 2":

• Are added to, or removed from, the accounts listed in this application, including CDF Transaction Accounts with a NAB Cheque Link Account
• I/we request CDF provides instructions to the National Australia Bank to complete these changes
• Are provided with, or have removed, CDF Online access to the accounts listed in this application, with the level of designated user access and online authorisation limit as designated in this application
• New signatories can enquire and give instructions in relation to cheques, international payments, withdrawals, periodic payments, direct debits, and where nominated, electronic services for both CDF and our banking partner, National Australia Bank (NAB), where the account has a linked NAB cheque account. These functions will be revoked for signatories being removed from the accounts listed in this application. 
• New signatories can give instructions in relation to Term Investments including opening, reinvesting, altering and redeeming. These functions will be revoked for signatories being removed from the accounts listed in this application. 
• New signatories can give instructions in relation to CDF Loan Accounts including requesting balances, interest information, payout figures and transaction history. These functions will be revoked for signatories being removed from the accounts listed in this application. 

Confirmation and Acknowledgement:

• I/we acknowlege that the Account Holder is fully responsible for the actions of all account signatories
• I/we acknowledge the Account Holder must make written requests to add or remove account signatories
• I/we acknowledge that this request is approved by the Account Holder and is in line with any governing documents such as Charters, Delegation of Authority or other documents. 
• I/we acknowledge CDF and NAB bear no liability for the actions of account signatories 

I/we confirm that each signatory will:

• Abide by the Terms and Conditions related to the use of CDF accounts, including Transaction, Savings, Loans and Term Investment accounts held by the Account Holder, including NAB Cheque Link accounts when applicable. 
• Abide by the Terms and Conditions related to the use of CDF Online.
• All information and documents provided by this individual are true and accurate. 

Terms and Conditions are available on the the CDF website www.catholicdevelopmentfund.org.au and are subject to revision and change at CDF's discretion - all changes will be updated on our website.
